A 7" vinyl with two songs, side 1: R.A.M.O.N.E.S. (Live), side 2: God Save The Queen (Live). On the front it says that R.A.M.O.N.E.S. is Live from Düsseldorf 2004 and that God Save is Live from the German talk show Harald Schmidt from 2000. So this means the Ramones song is probably taken from the Stage Fright DVD. I am not sure if the Harald Schmidt recording have been released before or if this actually is the first time it has been released on vinyl or CD.

The sleeve has a picture of Lemmy on front with title (Punk) and songlist while the back is just plain white. Vinyls are available on black or clear. 10 copies made, individually numbered (top left corner on front). First time I saw it was on Ebay in October2012, sold by Swedish seller, 666psycho666.
